Covers for cryoboxes

Dear List,
	I would like to make transparent plastic covers for cryoboxes.  The 
covers should be 13 mm in diameter and 1/16" (~1.5 mm) thick.  Does 
anyone know where to order these in lots of ~1000 for a minimal amount 
of money?  TIA.
